Most deferrals are timing failures, not competence failures: patient feedback left to year five, a colleague questionnaire that came back with nine responses, a quality improvement project never written up. Laid out against the cycle, those gaps are visible three years early. This tracker puts the GMC's six types of supporting information on one sheet with a column per year, and gives each type its own log underneath.

What's in the template
- Cycle tracker: the six supporting information types, the health and probity statements, indemnity, PDP and connection check, against years one to five, with the GMC requirement noted on each row
- CPD log: activity, the Good medical practice 2024 domain it evidences, hours or college credits, and the reflection your appraiser will ask for
- QIA, events and complaints log: quality improvement activity, significant events, compliments and complaints, with your role, the learning, and the appraisal it was discussed at
- Feedback exercises: questionnaire, numbers distributed and returned, themes and actions, for the patient and colleague exercises the GMC requires at least once per cycle
How to use it
Start the cycle tracker at your first appraisal after revalidation and put the date of each year's appraisal in the top row. Schedule the patient and colleague feedback exercises for year two or three, not five: the GMC's own questionnaires need around 34 patient and 15 colleague responses for a reliable picture, which means distributing to about 45 patients and 20 colleagues and waiting. Log CPD against one of the four domains as you go, so that by year four you can see which domain is thin. Keep the evidence itself in your appraisal platform or a folder referenced from the last column; the GMC expects you to retain everything discussed at appraisal since your last revalidation.

Who must keep this record
Every doctor with a licence to practise, and since December 2024 every GMC-registered physician associate and anaesthesia associate. The duty to collect and reflect on supporting information across your whole scope of practice comes from the GMC's revalidation regulations and its Guidance on supporting information for revalidation (31 January 2024). Doctors without a designated body or suitable person also need it for their annual return to the GMC.
How long to keep it
At least until your next revalidation. The GMC says you may be asked for the supporting information discussed at any appraisal since your last revalidation by a new appraiser, a responsible officer or the GMC itself, and if you previously had a connection that includes appraisals carried out by the designated body.
