The NMC gives you three years to collect 450 practice hours, 35 hours of CPD and five pieces of feedback, and then asks you to declare you have done it. Nothing is uploaded when you apply. If you are one of the applicants selected for verification, you have 21 days to produce the evidence. This workbook is built so that the running totals are always visible and the evidence reference sits next to every entry.

What's in the template

  • Checklist: all eight requirements with the target figure and a column for the evidence reference
  • Practice hours log: dates, employer name and address, setting, scope of practice, which registration the hours count towards, and a running total against 450 (or 900 / 1,350)
  • CPD log: method, topic and relevance, the part of the Code it relates to, hours, participatory hours, evidence held, and running totals against 35 and 20
  • Feedback log: source, type, non-identifiable content, and what you changed as a result
  • Reflective accounts index: which of your five accounts is written, on what, against which part of the Code, and when it was covered in your reflective discussion

What it does not replace

Three NMC forms are mandatory: the reflective accounts form, the reflective discussion form and the confirmation form. You must use the NMC's own versions of those, available from the NMC forms and templates page. This workbook covers the three logs the NMC describes as recommended (practice hours, CPD and feedback), where any record holding the same information is acceptable. The index sheet simply tracks that the mandatory forms are done.

How to use it

Fill in the practice hours log as you go, ideally monthly from your rota or payslips rather than annually from memory; agency and bank staff with several employers find this the hardest requirement to reconstruct. Log CPD on the day, with the certificate or notes filed against the evidence reference, and be strict about the participatory column: the NMC checks the 20-hour line hardest, and an e-learning module done alone does not count towards it. Never record anything that could identify a patient or colleague in the feedback log; a summary of the feedback and what you did with it is what the NMC wants to see.

Who must keep this record

Every nurse, midwife and nursing associate on the NMC register, every three years. The requirement to hold the evidence comes from the NMC's revalidation requirements rather than a statute, but the consequence of not being able to produce it is the same: without a completed revalidation your registration lapses and you cannot practise until readmitted.

How long to keep it

Until your next revalidation at minimum, so that verification can be answered within its 21-day window. Many registrants keep two cycles, because a confirmer or a new employer may ask about the previous one.
